Getting a headache! Online form

Hi Guys,

I am having some serious trouble trying to get this to work..... I am trying to implement a request a quote page on my website using Contact Form 7 on a wordpress CMS.

Something similar to this below: http://tier1logistics.co.uk/wp-content/uploads/2015/09/contact-form-img.jpg

Can anyone help me I am trying and trying and missing something I guess.
